       . Completed application form – you can download a copy of the form here
       . A copy of your trade license
       . A copy of your passport & authorisation letter if the applicant is other than the owner
         (if you are not an existing du Enterprise customer)

Once your 800 number is allocated, you will be contacted by du after which you can either visit any of our du shops or contact your du Account Manager to submit the formal application and documents.
